I think, she is some kind of acrobat/ballerina whose main act is something with pole - maybe she cant do her act cause she cant move, hence the benefit of volatren. The weird midget circus leader is not necessary as well as the little dog - but weak indeed.

this is just like the sensitive teeth toothpaste ad where you show a man/woman/dog grimacing with pain, and cartoon thunderbolts flying off the side of their face

Voltaren is used for curing (temporary of course) arthritis and rheumatism. Just can't figure it out why use the circus pips on this ad.
