I think the placement plays a big part... I'm pretty sure the ladies will go over every item if they're in the market for b&w fashion stuff, and be surprised by the dog.

I see nothing confusing in this. It's not overloaded.
But: This won't upset anyone. If a typical i.e. Vogue reader would see this ad without the copy, they would probably take it for absolutely normal xD

edit: Ok, maybe that's just it's point. If so: Well done.
